Step 1: See a Licensed Healthcare Provider

You must see an Illinois-licensed MD, DO, APRN, APN, or PA who can certify your qualifying condition. Both in-person and telehealth appointments are accepted. The provider enters an electronic certification into the IL Cannabis Tracking System.

Telehealth consultations typically cost $99–$200 and can be completed from anywhere in Illinois. Many telehealth services offer same-day appointments.

Step 2: Apply Online

Once your provider submits the certification, apply at medicalcannabispatients.illinois.gov. You will need:

  • Government-issued photo ID
  • Proof of Illinois residency
  • The electronic certification (already in the system from your provider)
  • Registration fee payment

Step 3: Get Provisional Approval (Next Day)

Applications submitted before 6:00 PM CST receive provisional approval the next business day. This is one of the fastest turnarounds in the country. Provisional approval allows you to purchase at any Illinois dispensary while your permanent card processes.
